4. Any title but must be *FOUR WORDS*
4. Pick one theme about death and *USE* that phrase in your poem

3. in the midst of life
4. love does not die
5. Required words - use all
1. melancholy
2. beautiful
3. thirsty
4. music
6. Add the date written, contest name, sponsor and *THEME* at the bottom.

Check spelling, grammar, punctuation if used, syllables, rhyming. Make sure you are writing in the form you selected correctly. And make sure your poem makes sense. You may add images, audio, video, if you want.
