Tetractys
Invented by Ray Stebbing, it is a 5 line poem with the 1st line having one syllable, the 2nd line two syllables, the 3rd line three syllables, the 4th line four syllables and the 5th line 10 syllables (1,2,3,4, 10) You may also(10,4,3,2,1) or a double Tetractys (1,2,3,4,10, 10,4,3,2,1)

Beautifully sad Emile, sad when someone suffers with chronic pain, but dying may be a release of sorts but brings other deeply sorrowful consequences along with it. The contrasting emotions are well described, I think this will be a winner!
