The pivot: the last

As part of the first phrase: a noun phrase composed of a determiner and an adjective (meaning the last thing or person …).
As part of the second phrase: the determiner (the) and the adverb last (meaning previously).

The Last Leaf

dry tree branch sways 
   autumn leaf clings
               the last
leaf through photo album 
   memories of friend persist
