Never-Always-Sometimes

Questions and more questions can often get us upset.
Finish this short survey for us, and a gift you will get.
Let us begin with Question #1. Ready, are you all set?
In your household, how often do you bathe your pet?

NEVER  O          ALWAYS  @           SOMETIMES O



Now for question #2. This one is all about romance.
Going on a blind date is considered taking a chance.
After having dinner, and depending on circumstance,
If you go to a club, does your date ask you to dance?

NEVER  0          ALWAYS  @            SOMETIMES 0



Question #3.  If dinner and dancing went really well
and you think you are falling under a romantic spell,
you share a sweet kiss and hear the ringing of a bell.
Are you one of those gabby people who kiss and tell?

Now for the final question. You must be glad only four.
Please answer truthfully although this surely is a bore.
