Here lies Jodie Catherine Lynn Fox
Slipped and fell in a patch of phlox
Hit her head on a stone
There she lay silent prone
We put her remains in a new frocked box

I think it might be Dorothy Parker, although several epitaphs have been attributed to her. Another funny one is "Gone, best forgotten" which is a twist on the expected. Thanks, Marco.
