Feb. 6, 2021 Hello I was born normal on May 17, 1979 from Paranaque, Philippines. My name is Jacqueline Ramirez Mendoza and was taken from Jacqueline Kennedy Onasis. Given by my cousin, Jocelyn Picazo Santos. My late father who passed away last November 25, 1986. My loving mother is with me right now, her name is Dorothy Ann D. Ramirez. I went from Grade 1 & 2 in Bauan East Central School until I moved to a Catholic School named blessed Elena Academy with Nuns in Pasay City Metro Manila. In there I joined the choir and received best leader award because I used to sing the Philippine National Anthem, the pledge of Allegiance and a prayer before we start our school day. I became a head cheer leader where I lead all the grade school level. I was 1st Year in HS going to 2nd when I migrated United States. We stayed with my grandmother, Aunt and her family in the city of Pacifica, California. I attended Terra Nova H.S. where I became a head cheer leader from 1st year to 2nd year. Then we moved to Daly City from mid 94-96. We stayed with my mother’s late co-worker who also passed away. Sometime on 1996 we moved to my grandmother’s friend in here at 124 B San Diego Avenue by Daly City Bart station. I also joined a choir with Anima Christi in St. Andrew Catholic Church, Soprano 1 where we sang in SF downtown with me as one of their soloist. I was a member of Fil-Am Multimedia Entertainment (FAME) directed by the late Jesse Pangilinan who passed away as well. It was October 11, 1998 when we had a concert of Outrage on Stage launching my one and only recorded song and I sang around the bay area. I started attending Westmoor High in Daly City where I graduated class of 1997.I originally attended The College of San Mateo but also attended Skyline College as well. As I remembered My ex-fiance, bf at that time and I fought, the reason I slept over with my best friend Adiel Rallos. It was October 10, 1999 where we were driving around San Mateo and Foster City. Went home at mid night and slept. Until the date of October 11, 1999 we woke up went to the College of San Mateo. My best friend had an interview at Fisher Investment where I’m all finished with my ex-fiance on the interview- we were to work together in Fisher Investment company in Woodside, California. Sometime around 4pm, I receive a traumatic obstacle of my life. I had a tragic car accident where my best friend who passed away 4 minutes after the collision and my left arm was crushed to 41 pieces due to her driver’s chair on her Blue Toyota and Stanford ICU inserted a plate, a rod and a number of screws according from the police report. When I was 1/8 awake my mother, family and extended family moved me from Stanford ICU to Seton Medical, Daly City. I guess sometime 2001 was when Seton Hospital was when the Orthopedics doctor took out the plate, rod and a number of screws. The prognosis at Seton Hospital was not good. I moved to St. Mary’s Hospital in SF. In there I had Occupational Therapy (OT), Speech Therapy (ST) and Physical Therapy (PT). I attended the Adapted PE in Skyline College with the instructor, Professor Chip Chandler. I slowly recuperated with Manual Wheelchair, Power Wheelchair, Walker, Quad cane and what I have now, a regular cane. While I was living with my ex-fiance and his family we did an in home physical Therapy with my ex-fiance and his brother, Arnold. I was supposed to be in a wheelchair but I don’t know what happened that I’m walking with a quad cane at that time. I remembered of me falling in front of building 7 at Skyline College. I would go on and off to True Hope, North County, Daly City from the year 2009 where I met Mr. Greg and Ms. Jackie Leeper. It’s not me because I didn’t know that I was so depressed that I fought with a few people and The College of San Mateo. I believe I started going to North County on weekly basis on 2013 when I met Dr. Chito Crisostomo and have him as my Psychiatrist.
