Never Sincere

"How do you do it"
they whisper to me
"Oh, I don't know" 
I smile and say

but

I know
I hear
I feel
Every hour,
cut,
skipped meal

Each day a reminder
they don't care to hear
so 
I smile
I giggle
but it's not sincere
